I don’t see how it could be real - the teeth look really misplaced for where the mouth is.

The corners of the mouth on most people are generally in line with where the teeth come together. You can clearly see from the “piercing” that this is not the case in this picture. Even if the mouth was “pushed up” by the piercing it would bulge in the middle where the piercing is and not move the whole mouth up evenly like it appears here.

Measure from the nose to where the top teeth allegedly reach down to. There’s no way that’s real.
